This sculpture is a portrait of a friend who suffered from severe depression.   He would often take on this expression when he became completely disinterested with life. The strength and largeness of his hand in this sculpture contrasts directly with the expression on his face, from his melancholic state.  It is merely the instinct of survival that holds him upright.
